With a Municipal Referral
List of required documents:
1. Referral to a residential care institution.
2. Application submitted by the individual or their legal guardian.
3. Agreement on accommodation in the care institution (agreement form).
4. Disability certificate, a copy of the work capacity assessment certificate, and copies of special needs assessment certificates (Forms SP-1 and SP-2).
5. Identity document: passport, identity card, birth certificate, or temporary residence permit in Lithuania for citizens of non-EU countries.
6. Certificate of declared place of residence or confirmation that the individual is included in the register of persons without a declared place of residence.
7. Extract from medical records (Form F 027/a), including the conclusions of the Medical Advisory Commission.
8. Assessment of the individual’s need for social care, including attachments.
9. Municipal certificate specifying the amount payable by the individual for social care.
10. Decision on the provision of social care (Form SP-9).
11. Certificate from the Real Property Register.
12. Court ruling, where the individual has been declared legally incapable or has limited legal capacity.
13. Reimbursable Medicines Passport.
When Entering into an Agreement Directly with the Institution
